Job Description:

Provides comprehensive clinical and psychosocial assessment and intervention for patients with serious illness.  Works independently and alongside other members of the multidisciplinary care team to conduct and participate in family conferences, particularly when there are complex psychosocial needs.  Consults and educates staff regarding social work practice, palliative care and end of life issues. Provides clear communication, both written and verbal, to the multidisciplinary staff regarding patient care concerns and end of life issues. Collaborates with appropriate clinical staff regarding patient and family centered case management; identify high-risk psychosocial factors of patient/families that impact health status and decision making. 

Required Job Qualifications:

300 Social Work hours under direct supervision of LCSW preferred

Master of Social Work from a program accredited by the Council on Social Work Education

Licensed Clinical Social Worker/Licensed Medical Social Worker (LCSW) preferred

Preferred Job Qualifications:

3 years Medical Social Work or Case Management experience with 2 years Oncology experience/knowledge

Active member of the National Association of Social Workers or Association of Oncology Social Work
