Position Overview

Documentation assessment and review of new patient medical record history.  Abstractors will document, in chronological order, all clinical history relevant to the patient’s cancer diagnosis.  This includes, diagnostic, treatment and social history.  Accurate abstracting of clinical data to meet regulatory and compliance requirements.  In absence of appropriate documentation, will seek out or query physicians for complete clarification.  Assist management with special projects as needed, which may include training and education.  Abstracting will work hand in hand with New Patient Intake’s – Intake Coordinators – to verify and complete an accurate patient history.

Key Position Qualifications

  • High school degree or equivalent and graduate of an approved Health Information Technology /Management program with certification as Registered Health Information Technician (RHIT), Registered Health Information Administrator (RHIA), RN or LPN certification
  • Minimum of 2 years hospital-based inpatient, observation and outpatient coding experience preferred.
  • DRG and APC assignment analysis to accurately reflect the diagnosis/procedures documented in the medical record
  • Documentation assessment and review for accurate abstracting of clinical data to meet regulatory and compliance requirements.
  • Experience with 3M or Ingenix Webstrat coding and abstracting systems preferred.
