The RN Patient Navigator functions as a member of the multidisciplinary team as an advocate and educator for patients from the initial phone assessment throughout the continuum of care, including diagnostic studies, diagnosis, treatment planning, treatment and survivorship. The RN Patient Navigator coordinates the new patient interview process, provides clinical assessment services to referral sources, families and patients to determine the appropriateness of evaluation and treatment at Cancer Treatment Centers of America. The Navigator assesses for special travel and medical needs. Builds a relationship with patients and physicians to coordinate a plan of care including appointments, education, provision and/or enablement of support services and representation within the multidisciplinary care environment. Assumes responsibility and accountability for the management of resources to achieve efficient, high quality outcomes for each cancer patient including support for interdisciplinary and cross facility collaboration. Ensures appropriate coordination of care between the patient, family, the multidisciplinary team members and outside agencies to ensure continuity of care and optimal clinical outcomes and resource management. The RN Patient Navigator acts as an advocate and point of contact for the patient and caregiver to ensure appropriate management of the patients healthcare needs, providing expertise in disease and symptom management to the patient while at Cancer Treatment Centers of America and at home.
Key Position Qualifications
- Current RN licensure for the state from an accredited college
- BLS/CPR certified
- Associates Degree in Nursing; BSN preferred
- Case Management certification preferred
- 3 years clinical oncology experience
- 1 year experience in Case Management and/or similar role
- Current ONS Chemotherapy/Biotherapy provider certification or attain during orientation
